VMH Bracing LLC (VMH) is committed to treating and using your protected health information in a responsible manner. This notice describes the personal information VMH Bracing collects and how VMH Bracing uses and discloses this information.  It also describes your rights as they relate to your protected health information. This notice is effective 04-13-2003, and applies to all protected health information as defined by federal regulations. VMH Bracing keeps a record of items provided to you. This record may also include information provided by your physician about your diagnosis, your medical history as well as your personal and insurance information. Your record is stored in VMH Bracing’s private and confidential computer system and files, and may be used in the following capacities: (I) assist health professionals who are responsible for your care; (2) legal reference describing the care you received; (3) a source of information for public health officials charged with improving the health of this state and nation; (4) basis for planning your care and treatment; (5) data source for medical research; (6) means by which you or a third-party payer can verify that services billed were actually provided; (7) source of data for VMH Bracing’s planning and development; (8) a tool with which VMH Bracing can assess, measure, and continually work to improve the care rendered and the outcomes achieved; (9) instruction tool for VMH’S Bracing to educate staff members; understanding patient records and how  health insurance operates helps  VMH Bracing’s staff ensure accuracy and make  informed  decisions.

 Although your health record is the physical property of VMH Bracing LLC, the information belongs to you. You have the right to: (I ) obtain a paper copy of this notice upon request;(2) inspect and copy your health record as provided for in 45 CFR 164.524; (3)amend your health record as provided in 45 CFR 164.528; (4) obtain an accounting of disclosures of your health information as provided in CFR 164.528; (5) request communications of your health information by alternative means or alternative locations; (6) request a restriction on certain uses and disclosures of your information as provided by 45 CRF 164.522; and (7) revoke your authorization to disclose health information except to the extent that action has already been taken.

 VMH Bracing has a responsibility to: (1) maintain the privacy of your health information; (2) provide you with this notice about VMH Bracing’s legal duties and privacy practices with respect to information collected and maintained about you; (3) abide by the terms of this notice; (4) notify you if VMH Bracing is unable to agree to a requested restriction; (4) accommodate reasonable requests you may have to communicate health information by alternative means or to alternative locations.   ·

 VMH Bracing reserves the right to change practices and to make new provisions effective for all protected health information maintained. Should VMH Bracing's information practices change, we will mail a revised notice to the address you or your physician supplied.

 VMH Bracing will not use or disclose your health information without your authorization, except as described in this notice, and as permitted by State and Federal law. VMH Bracing will discontinue use or disclosure of your health information after receiving a written revocation of the authorization.

 Examples of how VMH Bracing may use or disclose your health information follow:( 1)treatment–VMH Bracing may discuss your condition and/or treatment with your physician, your physician’s staff, other orthotists or prosthetists, VHM Bracing’s assistants, or the assistants of other orthotists or prosthetists.;(2) payment-VMH Bracing may send a bill to a third party payer or, if necessary, an agency specializing in collections if your account goes into default; VMH Bracing may also file your claim through a clearinghouse or via a secure website on the internet; (3) improvement of quality of service VMH Bracing may use your information to train staff; this information  is handled confidentially according to this Privacy Notice and State and Federal law; ; (4) communication purposes - when a Release of Information is provided, communication may be necessary with a family member or a person representing your interests who may be involved in your care or involved in the payment of your care; (5) research-VMH Bracing may disclose information to researchers when their research has been approved by an Institutional Review Board (]RB) who will also establish protocols to ensure the privacy of your health information;(6)notification purposes–VMH Bracing may contact you to provide appointment information or to discuss your bill;(7) Food and Drug Administration and/or the Department. of Health - relating to any product recalls, repairs, or replacements; (8) worker's compensation-VMH Bracing may disclose health information to the extent authorized by and to the extent necessary to comply with laws relating to worker's compensation or other similar programs established by law;(9)public health-as required by law, VMH may disclose your health information to public health or legal authorities charged with preventing or controlling disease, injury, or disability; (10) Jaw enforcement-VMH Bracing may disclose health information for law enforcement purposes as required by law or in response to a valid subpoena.

 Federal law makes provisions for your health information to be released to an appropriate health oversight agency, public health authority, or attorney.
