Patient Rights & Responsibilities

VMH Bracing, LLC encourages respect for the personal preferences and values of each individual and treats all patients with respect, dignity and consideration. You are entitled to these rights regardless of your sex, sexual orientation, race, culture, economic, educational, religious background or the source of payment for your healthcare. All your rights as a healthcare consumer also apply to the person who may have legal responsibility to make healthcare decisions regarding your healthcare.

 As a patient of VMH Bracing, LLC your rights include the following:

•      You have the right to receive unprejudiced care and to receive treatment regardless of your sex, sexual orientation, culture, economic, educational, religious background or the source of payment of care. You have the right to be treated with respect, dignity, and consideration.

•      You have the right to have knowledge of the name of the practitioner who has primary responsibility for coordinating your care and the names and professional relationships of other physicians and healthcare providers who will see you as a patient.

•      You have the right to receive as much information about any proposed treatment or procedure as you may need in order to give informed consent or to refuse the course of the treatment. Except in emergencies, this information shall include a description of the procedure or treatment the medically significant risks involved in the treatment, alternate course of treatment or non-treatment and the risks involved in each and to know "the name of the person who will carry out the procedure of treatment.

•      You have the right to participate actively in decisions regarding your medical care. To the extent permitted by law this includes the right to refuse treatment.

•      You have the right to full consideration to privacy concerning your medical care program. Care discussion, consultation, examination, and treatment are confidential and should be conducted discretely. You have the right to be advised as to the reason for the presence of any individual.

•      You have the right to confidential treatment of all communications and records pertaining to your care and treatment at VMH Bracing, LLC. Your written permission will be obtained before your medical records can be made available to any other medical or care providers.

•      You have the right to be free from all forms of abuse or harassment.

•      You have the right to know which rules and policies apply to your conduct while you are a patient.

•      You have the right to examine and receive an explanation of your bill regardless of source of payment.

VMH Bracing, LLC takes pride in providing its patients with the best care and service and encourages patients to provide comments about their care. Patient Responsibilities:

Along with your rights as a patient come certain responsibilities to ensure the highest quality health you deserve. As a patient of VMH Bracing, LLC you have the responsibility to:

•         Provide to the best of your knowledge accurate and complete information concerning your present complaints past illness and hospitalization and other matters relating to your health

•         Identify yourself as a person requiring language assistance services

•         Make it known whether you clearly comprehend your course of medical treatment and what is expected of you. You are encouraged to ask questions in order to have a clear understanding of any course of action and what to expect. If VMH Bracing, LLC is unable to answer your questions to you satisfaction your physician and/or practitioner will be notified to explain any questions you may have.

•         Report unexpected changes in your condition to you physician.

•         Follow both the treatment plan prescribed by your physician and supported by your practitioner and the product instructions.

•         Accept responsibility for your action should you refuse treatment or should you choose not to follow your physician’s orders.

•         Show consideration of the rights of other patients and VMH Bracing, LLC personnel at VMH locations with respect to your behavior. Please control the noise you and your guests make and do not smoke.

•         Show respect for your personal property as well as the as the property of others.

•         Assure your financial obligations for your healthcare are fulfilled as promptly as possible.

•         Comply with your physician’s order and do not permit any other individual to use your prescribed product.

•         Notify VMH Bracing, LLC promptly should you experience a product malfunction to enable appropriate product repair or replacement.

•         Understand that VMH Bracing, LLC is only able to provide you with an estimate on the amount your insurance company may pay for your product. Pay your bill according to your signed agreement regardless of your insurance coverage or insurance nonpayment.
